Select the Choice Options tab.

Enter Yes in the Flagged column next to the option you want to flagged when it is selected.

If you want a custom message to appear when the choice is flagged, enter the message in the Flag Message column.
Custom messages only appear when the option is flagged (the Flagged column is Yes).
