Use this dialog to create form note templates.
To open this dialog, from the Form Note Templates dialog, select New.
Form
Select a form from the list, or select 
to display a list of available forms from which to select the one that you require.
You can create more than one template for a particular form provided they have unique
names.
Template
By default, the template name is made up of the form name and the form number (up to a total of 20 characters). You can type your own unique template name.
Description
If a description exists for the selected form, it is displayed in this field by default. You can overwrite this text if required.
