Example: Let’s look at the provided sample Medical Equipment Risk / Inclusion Assessment.

Select the Medical Equipment risk assessment on the List View page.

Click the Decision Tree tab. Click on all the plus signs to expand the tree.

The Risk Assessment has five questions relating to it: Equipment Function, Physical Risk, Maintenance Requirement, Incident History and Management Override.

What is the Equipment Function of the asset?

What is the Physical Risk if the asset were to fail?

What is the Maintenance Requirement of the asset?

What is the Incident History of the asset?

Does management want to include asset in program even if score is below threshold?

Each question has answers with associated values. Each value is used in a formula for the assessment to determine a score.

Depending on where the score lies within a certain risk assessment index will determine the outcome of the risk.
Even if the calculations justify excluding an asset from the program, the last question
grants management the ability to override the calculation and force the asset into
the program.
